Oncology financial assistance

Quest Diagnostics understands that precision oncology testing may be overwhelming to some patients, and we are committed to helping them throughout their journey. As part of that commitment, Quest Diagnostics provides robust mechanisms for patients to afford oncology testing.

To give you in-network access to advanced testing options and trusted results, Quest partners with most health insurance plans, like UnitedHealthcare, Aetna®, Humana, Cigna®, and most Anthem® and Blue Cross® Blue Shield® plans—just to name a few. Quest Diagnostics is included in-network for over 92% of insured lives in the United States.

Regardless of whether your test is covered, Quest offers financial assistance that takes into account income, family unit size, testing completed, and extenuating circumstances.

If a patient has been approved for any level of financial assistance, their financial responsibility will either be the federal poverty level range percentage-based discount or a capped price for specific tests (whichever is lower).¹

Certain oncology tests within the oncology portfolio can have financial assistance opted into during test order using the following process:

1. The healthcare provider orders precision oncology testing, and

• You agree to allow Quest Diagnostics to access and use their demographic and other information to help determine if they are eligible for reduced financial responsibility for lab testing. This option is selected on the portal or on the requisition
• You agree to receive text/SMS messages about their precision oncology testing from

2. No forms to fill out—If your provider provides us with the necessary information to demonstrate your wish for us to use the information we have available to qualify you for assistance, no further financial documentation may be required

3. When required, Quest will work with your provider and request preauthorization for precision oncology testing, except in situations where the healthcare provider needs to provide this information due to your insurance requirements

4. If patient responsibility is likely, Quest will contact you with the details of the financial assistance options available to you

Quest offers payment plans with monthly installments for those who are unable to pay their full balance by the due date.
