Patient assistance program

We are committed to providing clinical laboratory services regardless of your ability to pay. Through our Patient Assistance Program, we tailor solutions for uninsured or underinsured patients based on individual circumstances and may adjust some or all laboratory charges if you cannot afford to pay for your testing.

Payment plans

We offer payment plans, which allow those who are unable to pay their full balance by the due date to make monthly installment payments.

Financial assistance program

We offer tiered discounts that take into account your income and family unit size. Discounts are based on guidelines provided by the US Department of Health and Human Services and can be as much as 100% of your amount due.

Eligibility

We will determine your eligibility based on your income and the US Department of Health and Human Services guidelines. The guidelines are updated annually and are available at the HHS website.

How to obtain assistance

To take advantage of a payment plan, call the customer service phone number listed on your bill. If you do not have a bill, contact Billing Customer Service.

To apply for our Financial Assistance Program, download an application and mail it to the address listed on your bill, or call the Customer Service phone number listed on your bill.

Financial assistance for Quest hereditary cancer solutions

Quest Diagnostics offers a reimbursement support program for patients who meet the necessary criteria for hereditary cancer testing. Upon receipt of a fully completed order and a patient blood sample, our team will verify coverage of your patient’s plan and quickly determine their likely out-of-pocket responsibility.

In the unlikely event that your patient’s estimated responsibility is over $100 for the Hereditary Breast Cancer Panel or Comprehensive and Guideline-Based Hereditary Cancer Panels, we will attempt to notify you and/or your patient prior to test initiation to discuss options for continuation or cancellation of the test. Please note that cases lacking complete information or a patient blood sample will not be processed, except where required by certain commercial insurance carriers.

Financial Assistance Program

Quest offers an easy-to-use financial assistance program that limits out-of-pocket expense to $200 for qualified patients with household income at or below 400% of the federal poverty level for qualified tests.

For uninsured and underinsured patients whose income is equal to or below the federal poverty level, testing may be provided at no charge. For patients who require additional financial assistance, Quest Diagnostics offers payment plans of 0% financing for a 12-month period.
