Understanding Testing Costs: Indiana's 50 Most Shoppable Tests
At Quest Diagnostics, we believe in empowering our patients with clear, upfront information about their healthcare. In accordance with Indiana's House Bill 1003 (HB 1003), Indiana has provided a list of the 50 most "shoppable" lab tests to help you make informed decisions about your care before you visit our testing centers.
A "shoppable" test is a published list of 50 specific services designated by Indiana as shoppable laboratory services. Because you can plan for these tests, this list gives you the opportunity to see the lowest and highest negotiated rate between Quest Diagnostics and any third-party payer as well as the patient out of pocket cost with no insurance.
Negotiated Rate: A negotiated rate is the rate that the laboratory has negotiated with any payor for that service.
Important Note for Insured Patients: If you are using insurance, your actual out-of-pocket cost will depend on your specific health plan's benefits, including your copays, coinsurance, and deductibles. A covered individual may at any time ask the health carrier for an estimate of the amount the health carrier will pay for or reimburse to a covered individual for nonemergency health care services that have been ordered for the covered individual or the applicable benefit limitations of the ordered nonemergency health care services a covered individual is entitled to receive from the health carrier. Indiana law requires that an estimate be provided within 2 business days. If you have any questions on the amount you may have to pay, please contact your insurance carrier directly.
Please Note: The negotiated rates listed here apply only to commercial (private) insurance plans. They do not include traditional government programs such as traditional Medicare or Medicaid, which have standardized government-set rates.
