Health risk assessments: Addressing deferred care and social determinants of health

Quest HealthConnect can efficiently complete thousands of comprehensive health risk assessments (HRAs) each year. Armed with predictive analytics and based on a member’s health history, our HRAs can help your plan get ahead of chronic conditions such as diabetes, cardiovascular disease, and chronic kidney disease.

Our HRA factors in social determinants of health to identify members at increased risk and help them take action to stay healthy. We can also provide lab work and other specialty services to meet each member’s individual needs.
