Quest offers flexible and easy-to-use financial assistance for both insured and uninsured patients. Our Supplemental Financial Assistance Program (SFAP) is a unique program available for QNatal and QHerit tests only, for patients who qualify. One of the following documents might be requested for verification: a copy of last year's W2 form, a copy of last year's income tax return, a copy of your most recent pay stub(s), or a proof source indicating that you are eligible for local, state, or federal assistance programs.
