Transparency in Healthcare Coverage

The Transparency in Coverage Rules require certain group health plans to disclose on a public website, information regarding in-network provider rates and historical out-of-network allowed amounts and billed charges for covered items and services in machine-readable files (MRFs).  The MRFs for the benefit package options under the Group Welfare Plan of Quest Diagnostics (Plan) are publicly available, and accessible to all without authentication.  The MRFs are not member-friendly consumer price transparency tools; they may not be easily interpreted or searchable.  The MRFs for the coverage options other than Kaiser are created and published for the Plan by the third-party administrators listed below; the MRFs for the Kaiser coverage options are provided by Kaiser.  To link to an MRF, please click on a URL provided below:
