This checklist can help confirm your heart-risk status with your doctor. Take this checklist with you and discuss at your next appointment. If 2 or more of the following apply to you, you are a candidate for our advanced cardiovascular tests.

Do you have a history of heart disease or a heart attack? (This counts as 2.)      
Do you have any other cardiovascular disease (such as peripheral vascular disease or vasculitis)?      
Do you have diabetes?      
Do you have pre-diabetes, insulin resistance, or any other type of metabolic disease?      
Do you have a family history of heart disease or heart attack before age 55?      
Do you have a total cholesterol of more than 200 mg/dL?      
Do you have an LDL cholesterol (bad cholesterol) level of more than 130 mg/dL, or an LDL cholesterol of more than 100 mg/dL if you have had a heart attack?      
Is your triglyceride level more than 150 mg/dL?      
Is your HDL cholesterol (good cholesterol) level less than 40 mg/dL?      
Is your systolic blood pressure (top number) more than 130 mm Hg?      
Is your diastolic blood pressure (bottom number) more than 80 mm Hg?      
Are you obese or overweight?      
Do you smoke cigarettes?      
Are you physically inactive? (Insufficient physical activity can be defined as less than 30 minutes of moderate activity 5 times per week, or less than 20 minutes of vigorous activity 3 times per week, or equivalent.)      
Do you have a history of autoimmune or other inflammatory diseases?      

Print and take these results to your next doctor visit to discuss next steps. Ask if there are other tests you should take for a more complete picture for your risks.
