Prepare for lab test sample collection

Schedule a sample collection appointment

To complete Quest Diagnostics lab testing, you need to visit one of our ~2,000 Patient Service Centers within the US to have your sample collected. If you don’t already have an appointment scheduled, you can easily make one online by visiting QuestScheduling.com or logging in to your MyQuest® account.

Please note that for any tests requiring at-home sample collection (not inclusive of Quest Mobile® patients), appointments are required to pick up sample collection materials and to drop off the sample.

At your sample pickup appointment, a Quest phlebotomist will provide you with sample collection materials, inclusive of detailed instructions.

At your sample dropoff appointment, please ensure your full name and date of birth are on each sample.

ATTENTION: If the healthcare provider who prescribed the lab work already supplied you with sample collection materials, you will still need to schedule an appointment for sample dropoff.

Prepare for your sample collection appointment

Abide by specific lab fasting requirements

  • Some lab tests require patients to fast for 8-12 hours
  • If you’re unsure if you need to fast, contact your healthcare provider to confirm. If you purchased a Quest test from questhealth.com, check the Prepare for your sample collection section on the specific test product page

Arrive early and with the right personal belongings

  • Arrive and check in at the kiosk 10 minutes before your appointment time to avoid a longer wait
  • If you check in more than 10 minutes late, you may lose your appointment. You will have the option to choose the next available appointment or standby
  • If you are visually impaired, use your mobile device to check in. Or when you arrive, swipe 3 fingers in any direction across our kiosk screen to check in
  • Bring a copy of your lab order if you have one. However, if your healthcare provider or employer sent the lab order directly to Quest, then we already have it and you do not need to bring a copy
  • Bring a valid photo ID for proof of identity, and your insurance card, if applicable
  • Bring your preferred method of payment if applicable. Should you have any overdue balances with Quest, we ask that you please pay before your appointment. Please note, if you need assistance paying for the tests you need, our Patient Assistance Program may be able to help

ATTENTION: During your appointment, you may be asked to sign an Advance Written Notice (AWN). Please note that health plans do not cover all healthcare services. In certain cases, for tests that Quest knows may not be covered based on health plan policy, we’ll issue you an AWN to notify you of your potential financial responsibility. Your phlebotomist will ask that you review and sign it if you’d like to proceed with the service(s). Look for more information about the AWN here.
