If culture is positive, identification will be performed at an additional charge (CPT code(s): 87118 or 87143 or 87149 or 87158). All isolates of M. tuberculosis will be automatically reflexed for susceptibility testing to INH, Streptomycin, Ethambutol, Rifampin, and PZA (CPT code(s): 87190 x5). Susceptibility testing of other species of mycobacteria will be performed on request.

Methodology

Decontamination and concentration as needed and microscopic examination by fluorochrome stain (auramine rhodamine). Conventional and broth culture methods with identification by DNA probe, biochemical or chromatography or sequencing.

Clinical Significance

Positive smears indicate the presence of acid-fast organisms and communicability. Isolation and identification of Mycobacteria is important for the management and treatment of tuberculosis and other Mycobacterial infections. There are more than 70 species of Mycobacteria of which the primary pathogen is M. tuberculosis. Tuberculosis is a chronic, granulomatous disease caused by M. tuberculosis. Identification and if appropriate, antibiotic sensitivity is required to initiate necessary therapy.
