appointment at one of the following Patient Service Centers:

1. The semen specimen should be collected after 2-7 days of
sexual abstinence unless otherwise instructed by the
ordering physician.
2. The specimen must be collected at one of the Patient
Service Centers listed above.
3. Patient should collect the specimen by masturbation into
a sterile container (containers are provided by the
laboratory). Since the volume of semen produced may be
significant to diagnosis, it is important to submit the
entire specimen. Be sure the container lid is closed
tightly.
4. Label the container with the patient name, date of
collection, and exact time of collection.

SAMPLE MUST BE COLLECTED IN THE MANNER AND LOCATION
SPECIFIED ABOVE, AND MUST BE RECEIVED IN THE LABORATORY
WITHIN ONE HOUR OF COLLECTION IN ORDER FOR MOTILITY TESTING
TO BE PERFORMED. SPECIMENS NOT MEETING ALL OF THESE CRITERIA
WILL BE REJECTED.

Room temperature.
Avoid exposure to extreme hot or cold temperatures.

Specimen Stability

Optimal - should be submitted to lab within 1/2 hour of
collection
