New Online Forms

This release includes a new online form.

Examples of online forms include Menu Ideas Form, Headache Diary, Exercise Planning Sheet, and First Appointment. They are identified by the form icon ( ).

These documents are in PDF format, which allows you to view, navigate, enter text, and print the form using free software called Adobe Reader from Adobe Systems, Inc. If the required version of Adobe Reader is not installed on your computer, you will be prompted with information on how to download a copy.

The information that you enter in these forms is not saved, so there are no security issues regarding the information that you enter. No one else will have access to any of this information.

• Immunizations: Immunization schedules. We now have a single Immunization schedule document that has links to all three childhood immunization schedules as well as the recommended adult immunization schedule.
