To get results sent to you, call our closest regional lab for the “Patient Request to Access or Disclose Laboratory Test Results” form or email our Privacy Officer to request the form. Fill out the form and return it to us. After we receive your completed form, you’ll get results within about 30 days.

You can also request your lab results by contacting your physician or using Microsoft® HealthVault®. You should check with your physician first to see if they are participating.

What is the Gazelle® mobile health app?
Gazelle® is our mobile health app from Quest Diagnostics that empowers you to see, store and share your vital health information. It enables you to store your health information including: doctor, insurance, emergency details, allergies, problem lists, vital statistics, prescriptions, lab results, and medical and immunization histories.

How do I sign up for Gazelle®?
First download the Gazelle® app. Then log in with your Quest Diagnostics username and password. If you don’t yet have one, create a password. Select "Request Results" from the "Lab Results" menu and fill in the requested information. The first time you use it, you’ll be asked to provide authorization to release your results and confirm your identity.

How do I sign up for Microsoft® HealthVault®?
First, create an account with Quest Diagnostics. Next ask your physician for a secure PIN for Microsoft® HealthVault®. Then follow the instructions to create the connection. You can do the same with all participating physicians. Once you connect to Microsoft® HealthVault® and your physician releases results, you can view them.

Does my physician have to participate in these programs for me to use them?
Physician participation is not required for Gazelle, but it is required for Microsoft® HealthVault®. Ask your physician if he/she participates in Microsoft® HealthVault®. If not, share with him/her our service overview, which outlines the program benefits for both practitioners and patients.
