Patient assistance program

We are committed to helping patients receive necessary clinical laboratory services regardless of their ability to pay. Through our Patient Assistance Program, we tailor solutions based on individual circumstances and may adjust some or all laboratory charges for patients who cannot afford to pay for testing. 

Financial assistance program

We offer tiered discounts that take into account income, family unit size, and testing completed. Discounts are based on guidelines provided by the US Department of Health and Human Services and can be as much as 100% of your amount due. Example tiers are shown below for illustrative purposes. 

  • Patients who meet or fall below the federal poverty level are eligible for a discount of up to 100% 
  • All patients who are at or below 200% of the federal poverty level qualify for a discount on amounts due to Quest Diagnostics for testing

For certain tests and in certain circumstances, patients up to 600% of the federal poverty level with extenuating circumstances may qualify for supplemental patient financial assistance.

Eligibility

We will determine your eligibility for financial and supplemental assistance based on your income, extenuating circumstances, and the US Department of Health and Human Services guidelines. The guidelines are updated annually and are available at the  HHS website. Financial assistance determinations are based solely on individual financial need.

Payment plans

We offer payment plans with monthly installment payments for those who are unable to pay their full balance by the due date.

How to obtain assistance

To apply for a payment plan, call the customer service phone number listed on your bill. If you do not have a bill, contact  Billing Customer Service

To apply for our financial assistance program, download an application and mail it to the address listed on your bill, or call the customer service phone number listed on your bill.
