Data Diagnostics® is a suite of real-time, member-specific data analyses that can be ordered on demand by your contracted providers at the point of care. Data Diagnostics is integrated into your provider network’s existing work flows to support improvement in clinical and quality outcomes in real time. Data Diagnostics can help to identify and address gaps in quality, risk, utilization, and medical history. It is seamlessly incorporated into providers’ workflows through integration with more than 400 supported EHR platforms. That makes it easy for providers to get the information they need, when they need it to help deliver cost-efficient care that keeps members well.

Three distinct categories of member health analytics

Clinicians can access member-specific insights within seconds:

Quality-Related Category: Provides a comprehensive picture of members’ current or predicted clinical and quality gap status for specific quality measurements (e.g., CMS 5-Star, ACA QRS, or NCQA/HEDIS®) and actions needed to improve quality.

Historical Data-Related Category: Provides insight into detailed medical history to help inform diagnoses, potentially reduce encounter time, avoid duplicative exams or tests, and aid in member evaluation and diagnosis for more efficient and improved care.

Risk Score-Related Category: Provides insight into member risk score accuracy gaps (e.g., HCC, CRG, or CDPS) and supports accurate disease burden documentation of unaddressed or worsening conditions to achieve a more accurate understanding of member-specific disease burden.

Her doctor orders a Historical Data-Related Data Diagnostics Report: Comprehensive Medical History Panel—One Year Lookback.

Within seconds,* the report lists past diagnoses, medications, laboratories, surgical procedures, vaccinations, radiological imaging studies, clinician visits, and cardiac studies over the past 12 months. Real-world impact: Assessing past, current, and future risk

Providers often have only limited insight into a member’s disease and comorbidity status. Moreover, they are often unfamiliar with the requirements for risk score coding. This can lead to inaccuracies in documentation, claims data and risk scores, which in turn can hinder reimbursement for health plans and accountable care organizations (ACOs).

The provider orders a Risk Score-Related Data Diagnostics Report: CMS Medicare Advantage Hierarchical Condition Category Risk Score Panel - Projected.

Within seconds,* the provider can now receive a report with member-specific insights into both historically-identified as well as predicated risk-adjusted diagnoses across a broad spectrum of applicable risk models to better support clinicians with member assessments, documentation, and risk score accuracy. The report identifies which risk-adjustment models the member belongs to and lists suspected risk-adjustable diagnoses based on information from pharmacies and labs, procedure results and other clinical data. The provider is now able to accurately document the diagnoses.
