Community & Giving: How to Request a Grant
Mission:
The Quest Diagnostics Foundation is committed to supporting health related charitable programs. Our focus is the funding of programs and projects that promote the prevention and early detection of disease or provide research for curing disease.
Guidelines for Requesting Grants
Quest Diagnostics Foundation is committed to supporting health-related programs (national scope) and projects (community based). Priority is given to funding programs and projects that promote the prevention and early detection of disease or provide research for curing disease. Only organizations established in the United States and classified as 501(c)(3) are eligible for grants.
Priority will be given to organizations that serve a geographic area in which Quest Diagnostics does business (Domestic and International). See Listing of Quest Diagnostics Facilities.
Grant Range
The Quest Diagnostics Foundation typically awards grants ranging from $500 to $10,000.
Restrictions
The Foundation does not award grants for:
- Individuals
- Conferences, seminars, tours, or related travel
- Fundraising activities including special events or videos
- Political causes and campaigns
- Political, fraternal, social or veterans' organizations
- Religious or sectarian organizations unless engaged in a significant project benefiting the community without regard to religious affiliation
- Organizations designed primarily for lobbying
- Benefit fundraising events or tickets for the same
- Counseling for substance abuse or family treatment programs
- Organizations that discriminate on the basis of race, gender, religion, culture, age, physical disability, sexual orientation or status as a military veteran
- Private foundations
Your organization will not be eligible for a grant if an employee of Quest Diagnostics Incorporated or one of its affiliates serves as a principal officer.
How to Request a Grant
All requests for funding should be in proposal form and must include the following information in order to be considered:
- A description of your organization and its mission
- A description and purpose of the project or program
- A description of how the outcomes of the program or project for which you are seeking support will be tracked (metrics)
- Amount requested from The Quest Diagnostics Foundation
- Specific community or geographic area served
- List of corporate/foundation donors
- Contact information (including e-mail address)
- Web address of organization (if available)
- Total costs and specific budget information for the program or project for which you are seeking support
The following documentation must also accompany proposal:
- Copy of your IRS determination letter indicating 501(c)(3) status
- Most recent audited financial report
- Annual Report (if available)
- Most recently filed Form 990
The Foundation Board reviews grant requests twice a year, in April and October. Grant proposals are accepted on an ongoing basis; however, all Grant proposals received by March 1 will be considered for April review and all Grant proposals received by September 1 for October review.
Due to the number of proposals received, we are only able to respond to those organizations that are approved by the Foundation Board in April and October.
